Duplicated TSM

I found issue that TSM page is totally blank 
So i delete this record from dbtools and I added TSM record Manually (I make sure that I have only 1 TSM record in F.TSA.SERVICE table)
the i cleared the TSA.STATUS and start the TSM 

but when I start TSM I found a lot of duplicate TSM Agents and duplicate agents for each service and this number of agents increased without any limitation until the server hangs 

how to fix this issue

is it a new installation? I never saw TSM record being blank...

Now - LOGOFF all tSA sessions, then:

CLEAR-FILE F.TSA.STATUS

Then try to start tSM again.

Post here your TSM record as well

Yes I already
1- LOGOFF all tSA sessions
2- delete COMO
3- CLEAR-FILE F.TSA.STATUS
4- restart the whole server

and issue still exist

I only have the attached TSM in TSA.SERVICE 
and you could see the duplicated agents
